‘Chicago Med’ Season 11 Episode 16 Recap: Did Dr Charles have a stroke? Major cliffhanger has fans worried

The latest episode of the medical drama series finds Dr. Charles overwhelmed after a series of stressful events, leading to his eventual breakdown.

'Chicago Med' Season 11 returns with a brand new episode this Wednesday, following last week's hiatus. The medical drama series is slated to air its penultimate episode, 'The Book of Charles,' on April 1 on NBC. Cherie Nowlan has directed the episode from a script written by Allen MacDonald. The Dick Wolf-created series, developed by Derek Haas and Michael Brandt, centers on the medical staff at the emergency department of Gaffney Chicago Medical Center. The latest episode focuses entirely on Dr. Daniel Charles (Oliver Platt), the hospital's Chief of Psychiatry, telling the story from his perspective. His own mental health takes a hit after a high-stress shift at the hospital, leading him to collapse at the end of the episode. 

Dr. Charles is seen experiencing a panic attack at the start of episode 16. The clock shifts back to 24 hours to showcase the series of events that led to his breakdown. While volunteering at the Greater Chicago Lifeline suicide prevention center the night before, Charles encounters a man named Gio on the verge of committing suicide. Gio tells Charles that he was in a car accident that led to his girlfriend's death and that he can't stop reliving the experience in his head. He shares that he was driving back from a holiday party with his girlfriend, Darby, and sped up the car as she was very drunk and started feeling sick. As he came close to hitting a woman on the crosswalk, Gio veered the car and crashed into a traffic pole. Sadly, Darby died on the spot.

Charles tries to reason with him by explaining that our brains slow down such events, tricking us into thinking that we could have changed the outcome. But Gio is not in a state to understand and cuts the call after informing Charles that he has a gun in his hand. Charles tries to reach him again, but his calls go unanswered, leaving him worried for Gio the entire day at work. He uses Atwater’s help to track down Gio, but it leads nowhere. A female patient, Sage, arrives in the ER complaining of severe abdominal pain, despite test results confirming there’s nothing wrong with her. After speaking to her estranged sister, Charles diagnoses Sage with Munchausen syndrome, a condition in which patients exaggerate or fake their physical symptoms to gain attention or care. However, Sage gets offended when Charles informs her and files an official complaint against him for verbally abusing her. Things take a turn for the worse when she swallows drain cleaner and is taken to the OR.

Charles gets wind of Lenox and Ripley's budding romance and Frost's growing feelings for Naomi, who already has a boyfriend. But it's his teenage daughter Anna's relationship that bothers him all day long. His concerns only grow when he finds out that her boyfriend, Jake, is a recovering addict who has been in and out of rehab recently. The episodes also show Ripley consulting Charles on his patient, Travis, who was seemingly sexually abused. However, the patient denies that he was abused and refuses to cooperate with the doctors. Ripley's efforts to convince him fall on deaf ears, and Travis leaves the emergency room against medical advice. Ripley later confides in Charles that he has started attending therapy to work on himself after his last two relationships ended for the same reason.

Meanwhile, Goodwin learns that Charles is being considered to take over his friend Howie Mankiewicz's job at Porter University of Medicine, but Charles assures him that he doesn't plan to leave his job. The episode then shifts to the opening sequence, where Charles is seen having a stroke. He hallucinates walking into his childhood home and meeting his estranged mother, Margaret. When he tells he he's dreaming, she asks, "You sure it's not a nightmare?" His fate is left unknown at the end of the episode, and fans will learn what happens in next week's season finale, 'Altered States,' which will air on April 8 on NBC. Viewers can also watch the episode the next day on Peacock.
