SAN FRANCISCO, CALIFORNIA: Dylan Mulvaney, the trans activist, received a letter from Vice President Kamala Harris on completing one year of transforming into a girl. The actress had also recently visited the White House to meet President Joe Biden to discuss trans rights and their issues.

What does the letter from Kamala Harris state?

Harris's letter claims, "Dear Dylan, I send you my warmest greetings as you celebrate your 365th day of living authentically. Thank you for courageously sharing your story and your journey. I appreciate your continued advocacy for transgender equality, including during your visit to the White House last year. Through your work as an activist and advocate for the LGBTQ+ community, you continue to break barriers and inspire young people across our Nation and around the world."

Further, it added, "While we have made incredible progress from advancing marriage equality to enacting historic protections for transgender people-the fight is far from over. Our Administration stands with the LGBTQI+ community in the ongoing struggle for true equity and equality and against hate and discrimination in all forms. I am grateful for your dedication and courage, and I hope you continue to use your platform to spread positivity and create change. I look forward to seeing all that you will accomplish in the future."

Did Dylan Mulvaney undergo any gender affirmation surgery?

Mulvaney has done facial feminization surgery which is one of the gender affirmation surgeries. She completed the surgery on December 16, 2022. "Facial feminization surgery encompasses a broad range of procedures to change the shape of the face to look feminine. Examples include having the hairline moved to create a smaller forehead, having lips and cheekbones augmented, or having the jaw and chin reshaped and resized. Skin-tightening surgery, such as a face-lift, is another option," as per Mayo Clinic. She wrote on Instagram back then, "Facial feminization surgery ✅ Huge thank you to @drharrisonlee who took such good care of me! Let the healing begin ❤️ #trans #ffs."

About the surgery, she said, "The surgery itself took multiple hours. It was the first time I was ever under anesthesia, and I don't remember a thing. I had a hairline advancement, a brow bone shave, a rhinoplasty, minor cheek enhancement, a little lip lift, a jaw shave, a chin reduction, and a tracheal shave," as per Entertainment Online. Further, she said, "FFS is sort of like a choose your own adventure," Dylan noted. "You can get one of those things. You can get all of them. You can get none of them. It's really whatever you have dysphoria over." The actress also added, "I went back to the doctor, where he injected me with steroid shots to bring down some of that swelling, and that was the first time that I looked at myself and thought, ‘Oh my gosh. I love what I'm seeing.' The craziest part is that it can take up to an entire year actually to see all the swelling gone," she explained. "So over these next few months, we will continue to see some changes but right now, here at the six-week mark, is a pretty strong idea of how it's gonna be."

Mulvaney also claimed, "This is an extremely privileged procedure and I want to stress that is not just typical plastic surgery," she said. "Had I not been trans, these wouldn't have been changes that I would have sought out. But because of my gender dysphoria, I felt I need it. And I want us to all start viewing FFS as just as important as other gender-affirming health care.Trans people spend years saving up for these type of procedures because they are rarely covered by insurance and it truly already brought me so much peace that I hope we can all start advocating for better trans health care."

What are the different types of gender-affirming procedures?

Vaginoplasty is one of the gender-affirming procedures. "In this technique, a vaginal vault is created between the rectum and the urethra, in the same location as a non-transgender female between the pelvic floor (Kegel) muscles, and the vaginal lining is created from penile skin," as per UCSF. Phalloplasty and metoidioplasty is another type of procedure carried out in transmen. As described by UCSF, "it involves the creation of a penis using any one of a number of procedures; either a free flap or pedicled flap of skin, usually taken from the arm (radial forearm free-flap, RFF) or anterior lateral thigh (anterior lateral thigh pedicled flap phalloplasty, ALT)". Another surgery is Masculinizing Chest Reconstruction. As described by Dr Esther Kim as "it will shape the skin and tissue of your chest to match the contour of a male chest."

Perioperative and postoperative care for feminizing augmentation mammaplasty is another popular gender-affirming care procedure. It is described as "an implant-based augmentation mammoplasty where a prosthetic implant comprised of a silicone shell, with saline or cohesive silicone filler, is placed underneath the breast tissue (subglandular implant) or under the pectoralis muscle (subpectoral implant)," as per UCSF. Gender affirmation care also includes voice care and voice transformation. There is also other care including tucking, binding, and packing. "Tucking allows a visibly smooth crotch contour, Packing is the placing of a penile prosthesis in one's underwear, giving both an outward appearance as well as reducing gender dysphoria, and Binding involves the use of tight fitting sports bras, shirts, ace bandages, or a specially made binder to provide a flat chest contour," as per UCSF.
