Dr Sarah Haller is contacted by a dancer who is too self-conscious to remove his shoes in public in the latest episode of 'My Feet Are Killing Me.' With a condition that is affecting his self-esteem, he seeks assistance from Dr Sarah. Patrick, who has been dancing his entire life, sends Sarah a photo of his toenail, which can only be described as having fungus below it. When Sarah looks at the photo, she believes she may have to remove all of his nails.

Dr. Sarah's patient, Patrick in 'My Feet Are Killing Me' (TLC)

Patrick describes his toenails as awful, to the point that no one, including himself, would want to see them. His toenails are quite dark and develop in a haphazard pattern. As a dancer, he had long suffered from just walking. He has had several injuries as a result of his childhood sports and dance, which have affected his feet. Patrick informs Dr Sarah that his nail has grown to the point that it is now throbbing into the flesh as if the shoe is biting back. He gets upset as a result of having to constantly cover his feet in public for so long.

Removal of the toenail

Dr Sarah notices Patrick's left foot is not in as poor a shape as his right. She asks whether he'd mind having his decaying nail removed, and despite his fear of the pain, he consented. Dr Sarah described his toenails as a "horn-like" structure, something that would go on a goat. Because of the "traumatic fungus", the nail that is removed returns, and is in worse condition. Sarah intends to treat it with medicine because he is a dancer who is supposed to be on his feet almost all day.

Dr Sarah intends to treat the left foot with topical and oral treatment, while the right foot will have its big toenail removed. She begins by numbing the region and removing the nail using an instrument. She feels that completely removing the toenail will allow it to grow back in good condition. To pull it off the toe, she carefully pushes back the cuticle, ensuring that the base to which it is truly connected comes off. After much struggle, the nail emerges from all corners.

Because his left foot's toenail was mostly flat, his right foot required extensive treatment at Sarah's office. The man with a low pain threshold goes through the session hoping for the remedy he has always desired. Dr Sarah puts Patrick on medication to ensure that the nail fungus is adequately treated. She also emphasizes that he must be diligent now and that the decaying toe nail may reappear with vengeance.
