When a man notices a gigantic mystery growth developing on his foot, he contacts Dr Ebonie Vincent, dreading foot amputation. In the fourth episode of 'My Feet Are Killing Me', Dr Vincent is tasked with the removal of the man's tumor while making sure that he does not lose his foot in the process.

Troy Monroe, who feels like he has a giant foot, has a massive bone tumor on his left foot. He describes it as a massive volcano that is boiling over and about to erupt. Troy's first appointment with Dr Vincent resulted in her sending a bone from his foot for biopsy. The test results were negative, but the tumor continued to grow, forcing him to return to the doctor.

Troy works as a security guard at a casino, which requires him to stand for 8 hours and take 20,000 steps daily. Things have become nightmarish for him with speech difficulties and occasional twitches. For Troy, walking home feels like breaking a bone. Since cancer had been ruled out, Dr Vincent's major aim is to remove the whole tumor and repair the fracture beneath it. Troy's accounts of the tumor becoming larger and tingling in his feet become the cause for alarm.

Troy wants Dr Vincent to keep his foot intact. The tumor is quite large, taking over a portion of the bone and pushing it down. Dr Vincent diagnoses Troy's ailment as a "growing bone tumor". The tumor has infiltrated all the nerves, particularly the dorsal cutaneous nerves of the foot. He has a Lisfranc fracture beneath the tumor, and the bone tumor is squeezing all the bones. Dr Vincent intended to make as few incisions as possible before fusing the region with a plating system and screws. However, she is concerned that even once the tumor was removed, Troy might be unable to walk again.

Finally, on the day of the surgery, Dr Vincent attempts to find the boundary between the tumor and the bone to raise it. She inserts an osteotome between the normal layer and the bone tumor layer to pry it up and snap it off. Once everything has been cleared up, the doctor repairs the fracture as well.

Troy returns to Dr Vincent for a check-up two months following the procedure. She had already sent the remainder of the tumor for biopsy to ensure that the tumor was benign and that there was no risk of foot amputation. As the results are negative, she advises Troy to make sure he does all she suggests so that her situation does not worsen. She eventually fixes Troy's foot, allowing him to regain mobility without having to amputate it as they all thought would happen.
