Steven Assanti and his brother Justin Assanti decided to do their journey together but things did not work out for the brothers as they decided to head their separate ways. While Justin seems to be doing good and focusing on his life forward, things have not been the same for Steven.

In the season finale of the show, the Assanti brothers returned. When they first appeared on the show, Steven had received a lot of hatred for the way he behaved. Fans were not happy with how he treated his brother and father. Steven's behavior had everyone questioning if he was fine. Some were worried that he might need help with therapy. Even though Steven managed to get approved for the weight loss surgery, things did not get better for him. Dr Younan Nowzaradan soon realized he had a pain killer addiction and was stealing them from his brother's prescription.

Despite being sent to rehab for a while, things did not seem to work out for Steven and Dr Nowzaradan, who is famously known as Dr Now, who gave him an ultimatum to get things under control. Now, nearly a year later, Steven and his brother returned on the show but it looked like Steven had not made the progress that Dr Now had hoped he would. Steven revealed he hadn't been talking to Justin and they had been staying separately. It seemed like the relationship between Steven and Justin had finally come to an end. The brothers even decided to meet Dr Now separately. Steven was the first person to go meet him.

He was certain that he might have lost a lot of weight as he had been working hard on himself. However, once he reached there, he found out that he had only lost 4lbs. Dr Now was not happy to see the weight loss and asked Steven what he had been doing. He admitted that he was having a hard time staying on the diet but promised to make sure he gets back on track. However, his weight was not the only worrying thing for Dr Now. Dr Now asked Steven if he knew what soap and water were. Dr Now did not hold back in letting Steven know that he did not smell good. He told Steven that there are people outside puking because of his smell.

Hearing this Steven just smiled and did not give an answer. Dr Now was curious to find out when was the last time he took a shower and Steven revealed it was the day before yesterday. However, Dr Now did not believe him and told him how important it was for him to take a shower. Steven also revealed he had an allergy near his stomach area and after looking at it Dr Now understood it was because he was not showering. Dr Now advised Steven to get back on track where he would be working out and taking proper care of his hygiene.

Steven agreed and left the hospital but Dr Now knew it was going to be a long road until he took things seriously.
