Spoilers for 'Suburra: Blood on Rome' Season 3

The joyous occasion of finally being able to give the Sinti Anacleti family an heir is marred by grief when Angelica (Carlotta Antonelli) miscarries her baby girl. In the beginning, Spadino (Giacomo Ferrara) was a little put off by the idea of having a daughter when he was sure it was a boy, but he came around to loving the baby just the same. Still, with vengeance lurking every corner, Angelica isn't able to carry the baby full term and their marriage subsequently also falls apart, even though not quite for that particular reason.

Angelica and Spadino commence their preparation for the baby in full swing, but it is Manfredi Anacleti's (Adamo Dionisi) return from the hospital that proves to be a hassle. Manfredi immediately picks up his rivalry with Adami Aureliano (Alessandro Borghi) and in a fit to teach him and his girlfriend Nadia (Federica Sabatini) a lesson, he ambushes them, twice, separately. The second time, Angelica is alone with Nadia when the latter is shot by Manfredi's men, and when Spadino finds out, he vows to avenge almost losing his wife and their baby. Spadino charges into Manfredi's bedroom and attacks him with a knife, but eventually withdraws and leaves the premises without killing his older brother. Manfredi however is not done avenging his accident.

When Spadino tells Aureliano he wasn't able to kill Manfredi, the latter vows to finish the job he started. Aureliano tails Manfredi's car and ambushes the vehicle with a barrage of bullets and crashes, almost totaling the automobile. When he appraoches the vehicle, however, he finds Manfredi inside with Angelica, holding a knife at her throat she Aureliano try to kill him. Manfredi had practically forced Angelica to get into his car before all this, so there was no chance of Aureliano suspecting anybody worth watching out for in the vehicle. The accident and attack however manages to leave Angelica distressed, with the crash causing her a miscarriage.

The miscarriage also leads Angelica to revaluate her and Spadino's marriage, considering he is gay, and no matter how devoted and caring they are, they will never be the right fit for each other in the long run. Noticing the damage he brings to Angelica's life, and not able to deny his innate feelings any longer, Spadino finally calls it quits at the end of the Seaon 3 finale.
