According to a study, the phrase "fatal Russian roulette" describes a fatal act of great bravery in which the perpetrator turns the cylinder of a revolver with at least one round inside, points the muzzle at the target, and pulls the trigger. Cambridge Dictionary defines Russian Roulette as "a very dangerous game of chance where each player aims at their own head with a gun that has one bullet in it and five empty chambers (= spaces where bullets could go)"

The Medical Examiners' Offices in Kentucky conducted a retrospective examination of self-inflicted head gunshot wounds over a ten-year period (1993–2002), with a focus on Russian roulette cases. According to the study Research Gate compiled, the average age of the 24 Russian roulette occurrences victims was 24.8 years, and 79.2% of them were white men between the ages of 14 and 47. The participants playing Russian roulette had a significantly higher chance of having increased blood levels of ethanol as well as different substances that could be found in urine as compared to other cephalic firearm suicides. Russian roulette is classified as suicide since it was essentially deliberate and voluntary on the part of the deceased, despite the risky act's presumed purpose being to survive.

In an incident that happened in 2019, a Michigan State University student who was on break in Chicago was fatally shot by her boyfriend in a game of Russian roulette. The prosecutors revealed that she was fatally shot by her boyfriend who was playing with the gun in an apartment on the city's Southside. A judge ordered Lafayette Hodges, 18, to be held without the possibility of bail. He faced a felony count of involuntary manslaughter in the death of Lyniah Bell, 19, who took a gunshot to the head.

As reported by MEAWW, after turning himself in, Hodges had reportedly shared with the police that 'he loved her too much to hurt her' and claimed that they had been 'play fighting' with the gun. Hodges had many run-ins with the law and had told the police that he believed that the gun was empty when it went off. While in a bedroom, Hodges had played with the gun and had first pointed it at a friend and pulled the trigger. He then did the same to himself after which he pointed the gun at Bell and then fired it. Hodges' family called 911 while he ran from the apartment with the gun.
