In the latest episode of 'My Feet Are Killing Me,' Season 4, Kali, a patient with a webbed toe approaches Dr Ebonie Vincent. Dr Ebonie's procedure strikes a snag as she tries to assist her patient out of her years of agony.

Kali, Dr Ebonie's new patient, who is seeking treatment for her "special toe", had to keep her disease secret from her loved ones and significant others because she had struggled with body image as a child. She has been so preoccupied with her toe that it has harmed her self-esteem. She finally decides to seek help from Dr Ebonie in the hopes of regaining normalcy in her life, who tries her best not to amputate her toe during the procedure.

Dr Ebonie's patient Kali in 'My Feet Are Killing Me' Season 4 (TLC)

Dr Ebonie understands Kali's insecurity, which is driving her depression and anxiety. Kali tells the doctor that she was born with a webbed toe on her left foot, which she had to deal with while growing up, becoming increasingly nervous about it over time. She says that, while she does not suffer from chronic pain, the chafing against the shoes causes her distress. Kali had her x-rays taken before meeting Dr Ebonie. Her pinky had two bones, according to the x-ray. Her exterior bone was ordinary anatomy, and the other bone she possessed was polydactyly (extra finger or toe). The extra toe is again webbed to the fourth toe, which is known as syndactyly (two or more digits fused together). The terms, however, did stump viewers of the show as they tweeted, "What I'm hearing is she is a pterodactyl @DrEbonieV #myfeetarekillingme."



 

Dr Ebonie was taken aback when she encountered the patient with both polydactyly and syndactyly for the first time. Dr Ebonie offered a complicated toe separation procedure that involves not only separating the toe but also removing the excess toe. Dr Ebonie could use the additional skin on the top and bottom of the toe to create two distinct toes once the bone is removed. Webbed toe separation procedures are never easy since ample space is required to cover and separate the toes. The surgery was complicated further by the need to extract the additional bone without compromising the skin in order to subsequently utilize the skin. Even though it was a little toe, the procedure was not small because amputation was a possibility. Despite the risks, Kali agrees to the cosmetic surgery treatment in order to reclaim her self-esteem. Fans on social media also cheered to have her extra digit removed. A tweet tagging Dr Ebonie read, "@TLC She has the perfect build! Get rid of that toe! She has got to feel better about herself!! #MyFeetAreKillingMe@DrEbonieV."

One of the main issues Dr Ebonie faced during the operation was a lack of blood flow, therefore she couldn't remove a lot of the soft tissues since they had tiny capillaries that give blood to the toe. Even the tiniest vessel has a significant impact on the tiny toe. Dr Ebonie admits to experiencing anxiety while operating on little toes since they are delicate and finicky. She had two hours between surgeries to perform the procedure and remove the tourniquet to ensure blood flow returned to the toes; otherwise, Kali may develop complications and the skin could die, culminating in amputation.

The tip of the extra toe was attached to Kali's fifth toe. To remove the additional toe, Dr Ebonie had to perform a precise cut at the sensitive region. The doctor stitched it up in the hopes of restoring blood flow, but to her dismay, the fifth toe begins to turn white. The presence of white toe signals that blood flow is not reaching the region, which can be problematic as the skin begins to perish. If the patient becomes infected, the doctor will be forced to perform an amputation. Dr Ebonie chooses to lower her foot off the table in the hopes that gravity would do the rest. Finally, the toe receives a much-needed blood supply. Despite the fact that the toe was pale, the doctor was relieved to discover that it did not remain white. Just like Dr Ebonie in the surgical room, viewers of the show had a brief panic moment. Some took to Twitter to write, "I had a feeling this was going to happen, too much focus on getting blood flow back #myfeetarekillingme."



 

Kali returns to Dr Ebonie for a check-up two months following the operation, with her confidence restored. The toe healed properly and resembled the other foot that had nothing wrong with it.
